Branch, M.D. will retire from the practice of medicine, on June 30, 2019. Each patient's medical records will be forwarded to a provider of the patient's choice, and to the patient if requested, upon receipt by University Orthopaedic Clinic of a fully completed release of information form (available at UOAtlanta.com) at the above address on or before June 30, 2019 or at the address for submitting a fully completed release of information form after June 30, 2019 set forth at UOAtlanta.com.
