NOTICE OF CLOSURE OF MC Dental LLC d/b/a Walmart Health, a Georgia limited liability company. MC Dental LLC will discontinue furnishing services in the state of Georgia no sooner than 30 days after the date of this notice. As always, patients have the right to obtain their medical records from the practice or its designated custodian. In accordance with applicable law, patients who wish to receive a copy of their medical records or who would like to have their medical record transferred to another practice or provider may make a request by email at records@walmarthealth.com or call 1-800-891-0315.
